Community Lead and Website Coordinator at CD Projekt Red Marcin Momot has confirmed that Patch v.1.12 for the Witcher 3: Wild Hunt will roll out later today.
Momot confirmed it via Twitter where he also mentioned that the Patch Notes will be released alongside the patch:

Speaking of the platforms on which Patch v.1.12 will roll out, Momot confirmed that it will release across all 3 platforms i.e. PC, Playstation 4, and Xbox One simultaneously:

The Witcher 3: Wild Hunt Patch v.1.12 was originally planned to roll out in December 2015; but CDPR pushed the release date to 2016. While the official Patch Notes will make things clear, Patch v.1.11 caused some issues with a couple of quests and Runesmith introduced in the game’s Hearts of Stone expansion.
Being on the best role-playing games of 2015, the Witcher 3: Wild Hunt is scheduled to receive its second expansion pack dubbed Blood and Wine this year. We also recently spotted the game’s Enhanced Edition which was quickly labeled as false by the developers saying that currently there are no plans for an Enhanced Edition of the game.
