With all of the free DLC for The Witcher 3: Wild Hunt now available thoughts for the players is turning to Hearts of Stone and what they get with the season pass. Marcin Momot answered some questions about the DLC and how to prepare for it.
When a user on Twitter asked at what point Heart of Stone can be accessed Momot, the Community Lead and Website coordinator for CD Projekt Red revealed:
@savroRus You can play it after/during the main story. The HoS quests will be level 30+ though.

So while Geralt will have to be levelled up to at least 30, you won’t have to have completed the game to take part in the new content. For people who are playing through the New Game + elements, he also confirmed that Hearts of Stone will scale up like the rest of the game, to heighten the challenge for the players:

Hearts of Stone as well as Blood and Wine are the two next expansions to The Witcher 3: Wild Hunt and are expected to add almost 30 hours to the game. These stories will take part in new areas that will be added to the game for the players to experience. Even though as confirmed above the player won’t have had to complete the game first, a good majority of fans have already done this. Chances are that most will begin with New Game +.
