Naughty Dog has announced that it is releasing The Last of Us: Left Behind DLC as a standalone title next month on May 12, 2015.
The announcement came via Naughty Dog Twitter Feed; the developer announced that the standalone expansion will be available both on Playstation 4 and Playstation 3 alongside a two-hour Factions Multiplayer demo:

The Left Behind standalone game will have built-in upgrade paths through the in-game store to both the full Factions experience and/or the full game for The Last of Us.
